Assistant Project Director, Health Care for the Homeless Program

Location: Health & Wellness Center | Clay City

Job Description:

Compensation: Exempt Position/40 Hours Per Week
$70,000 – $75,000 Per Annum

Position Summary:  The Assistant Project Director will facilitate the development, coordination, implementation and evaluation of patient care activities and clinical quality improvement initiatives within each medical team. This position will focus on areas such as efficient use of EHR (Electronic Health Records), meaningful use and other clinical quality outcome measures, Patient Centered Medical Home requirements and chronic disease management. The Assistant Project Director is responsible for clinic marketing, recruitment, and outreach activities. This position also serves as the on-site Risk Manager for the program and provides direct supervision of Care Coordinators, Chronic Care Manager, and the Referral Coordinator.
Education, Experience & Training:  Bachelor’s degree required in healthcare administration, public health, social services, or other health related field.  3 years minimum experience with quality improvement oversight in a health care setting strongly preferred along with administrative and management skills in personnel, clinical measures, marketing and advocacy. Must demonstrate commitment and sensitivity to the needs of an underserved homeless population.  Prior experience with a Federally Qualified Health Center (FQHC) or Rural Health Clinic (RHC) helpful.  Must be proficient in most computer applications, including Microsoft Office and NextGen/EHR, and have good oral and written communication skills.
