$21.00 to $23.00 Per Hour/Non-Exempt

Summary of Qualifications: Under the general supervision of the Clinical Director/ Nurse Manager/ Clinical Quality Manager, applies nursing skills in the care and treatment of patients while also conducting outreach to and advocacy for the homeless population. The Patient Community Education Specialist is responsible for overall health communication activities that promote healthy behaviors to improve wellness and disease prevention. The Licensed Practical Nurse will be responsible for providing quality care of patients in a safe, effective, efficient manner and to act as a liaison between clients and their families and health, mental health and social service providers.

Must be a licensed LPN (Licensed Practical Nurse) or RN (Registered Nurse) with two years’ experience in a clinic setting; CPR Certification required; familiarity with client population, knowledge of Community Action programs and local resources strongly preferred; must be proficient in most computer applications; including Microsoft Office. Excellent oral and written communication skills required and must have the ability to prioritize, plan, and organize work efficiently.

While performing the duties of this job, the employee is regularly required to talk or hear. The employee is occasionally required to stand; walk, climb, balance, stoop, kneel, crouch, crawl, and use hands to manipulate handle or feel; and reach with hands and arms. The employee is occasionally required to lift/and or exert force up to 50 pounds.
