Our goal is to be an extension of your home environment with quality, safe and personalized care. Each day Foothills Active Day Center is filled with activities to promote socialization, therapy, recreation, and relaxation.
Active Day Services emerged decades ago. It’s an alternative to institutional care when families need help caring for a loved one, but either cannot afford assisted living or personalized care for one at home. The program offers an alternative to nursing home care for those who do not need 24-hour skilled nursing. Active Day services often assist working caregivers to balance the needs of work and caregiving responsibilities as well as respite for full-time family caregivers.
Operated by Kentucky River Foothills Development Council, Inc., the Foothills Active Day program provides comprehensive health, social, supportive, and therapeutic services for functionally impaired adults in a community-based setting. Our program provides caregivers a break by offering respite care options five days a week. It’s also a valuable lifeline for someone whose goal is to keep their loved one in the home.
Since we began offering Active Day Services in 1999, we have helped hundreds of individuals and families remain independent. Our program is designed to help people stay mentally and physically active while reducing their isolation, improving their health, and preventing the decline of their abilities.
We serve clients that have a physical, mental, and/or intellectual disability as determined by an independent assessor. We primarily focus on providing care for people with a specific chronic condition, including Alzheimer’s disease and related forms of dementia; additionally, these services may be available for any adult with a disability.
Referrals are also received from Home Health Case Managers. Although located in Powell County, we serve clients from Powell and many neighboring counties, such as Clark, Estill, and Montgomery.
We are experts, providing not just care, but coaching and experience to help navigate every step of the journey. We are passionate about helping clients because we know we are making an impact in their lives.
Social Activities
Each day is filled with activities to promote socialization, therapy, recreation, and relaxation. Clients may come as often as they wish, or according to the needs of the participant or their caregiver. During their time with us, participants spend time with others and improve their daily living skills. Clients take part in supervised exercise programs, crafts, outings, gardening, birthday parties, holiday celebrations, and relaxation techniques.
Health Monitoring
Dedicated, skilled nurses and LPNs are always on-site while clients are at the center. During each day, clients are served breakfast, lunch, and snacks. Clients receive assistance with eating, taking medication, toileting, and walking. They also receive health monitoring for concerns such as blood pressure and blood sugar. Transportation, often through Foothills Express, is provided.
Preserving Independence
We offer a chance for people with dementia or other disabilities to get out of the house and be on their own in a safe environment. Clients enjoy control over activities that they partake in, boosting their self-esteem and sense of well-being. Our program can also help maintain positive relationships between the person and the caregiver.
Convenience for Caregivers
Families who take part in our program consistently experience lower levels of stress related to caregiving, report increases in socialization opportunities for their loved ones, experience decreased symptoms of depression, and benefit from cost savings versus in-home care. Caregivers receive much needed respite while their loved one is with us, and often go to work, take care of personal business, or simply relax.
Licensed Medical Care Facility
We are a licensed medical care facility through the Office of the Inspector General and Medicaid. Medicaid covers the cost of adult daycare for qualified patients. Participants that do not qualify for Medicaid can choose private pay.
When Being Home Is The Best Care
We understand that not all adults can be active members at our Center. We also provide home-delivered meals and attendant care in the clients’ homes. On-site attentive care helps clients with daily tasks such as personal care, light errands, and activities. We take pride in providing person-centered care, all while maintaining individual dignity and self-identity.
The main benefit of Active Day Services for adults is delaying or preventing the move to a long-term residential care facility so adults can live at home or in the community as long as possible. Benefits of an active day program can also include:

We are a licensed medical care facility through the Office of the Inspector General and Medicaid. Medicaid covers the cost of adult daycare for qualified patients. Participants that do not qualify for Medicaid can choose private pay. Patients/caregivers can choose a schedule that fits their needs and are charged per 15-minute units.
Transportation to the Foothills Active Day Center is available through Foothills Express.
Foothills Express provides non-emergency medical transportation to Medicaid clients in Clark, Estill, Madison, and Powell Counties. For information, scheduling, or eligibility, you must call Foothills Express’ broker, Federated Transportation Services of the Bluegrass (FTSB) at (859) 258-2722 or 888-848-0989. Medicaid paid transportation requires a 72-hour notice.
Construction of a new 5,000-square-foot building in Clay City is expected to be completed in the Fall to accommodate the growing membership for its social-based activities and respite program. Our new location will accommodate 35-40 people daily. This new Center is designed with the clients’ mobility in mind. It will feature outdoor programs for gardening, exercise, and art classes along with more space for indoor programs. In the outdoor garden, raised beds will be at different heights to be more comfortable for the participating gardeners.
Tours Available
We welcome new clients. No need to make a reservation, just show up, learn about our Center, and plan on staying for lunch! You will meet new people and participate in community events. Our goal is to become an extension of your home.
