KRFDC serves men, women and children through a community-oriented, primary care setting where they can address a wide variety of medical needs. KRFDC is the only recipient of federal grant funding to provide primary health care services exclusively for vulnerable homeless individuals and families in Powell and surrounding counties.

The most common primary diagnoses of adult individuals served by the KRFDC Health & Wellness Center include:

  • Hypertension Nurse
  • Diabetes
  • Obesity
  • Metabolic Syndrome
  • COPD (Chronic Obstructive Pulmonary Disease)
  • Depression disorders
  • Anxiety
  • Heart disease
  • Substance abuse
  • Hypothyroidism

Medical Services
KRFDC provides comprehensive medical services including exams, screenings, health education, and referrals for specialty medical care. Our medical care services include:

  • Physical examinations
  • Immunizations (limited)
  • Office and laboratory testing
  • Care for acute health problems
  • Treatment for chronic health problems

Staff also provide education to clients about health management and disease prevention.

Prescription Assistance
KRFDC maintains a Patient Prescription Program that provides low cost medications to uninsured patients who qualify. Clients pay a low-co pay (approximately $2) for the prescription medications provided through the program. KRFDC does not operate an on-site pharmacy, nor does any medication stay in the Health & Wellness Center.

Mental Health Services/Substance Abuse
KRFDC provides quality holistic behavioral health services to its patients.  HCH Mental Health Services Flier (PDF). Our mental health services/substance abuse services include:

  • Crisis intervention
  • Ongoing therapy and outpatient treatment
  • Case management and care coordination
  • Access to supportive housing
  • Referrals to other internal and external services, as appropriate
  • Outpatient Substance Abuse Counseling
  • Linkages with residential substance abuse providers

KRFDC believes that good oral health is part of an individual’s overall well-being and also contributes to self-esteem. Limited dental services may be arranged for patients. When a patient presents with a dental problem, a referral is made to the Frenchburg Dental Clinic.

Outreach & Supportive Services
KRFDC provides a full array of services for our patients’ beyond their health care needs. Our Outreach & Supportive Services include:

  • Crisis intervention
  • Case management and care coordination
  • Casework to secure food, shelter, clothing, and other emergency assistance
  • Ongoing therapy
  • Public benefits assistance
  • Housing location and maintenance
  • Information & referral
  • Nutritional counseling

Nutritional counseling is also available with a registered dietitian for diabetes, cardiovascular disease, and weight management.
