Foothills Active Day Center

Our Active Day Services have been serving families for decades, offering an alternative to institutional care for individuals who require assistance but may not need 24-hour skilled nursing. Our program offers comprehensive health, social, supportive, and therapeutic services in a community-based setting.

Who We Serve

Our program caters to adults aged 21 and above with physical, mental, or intellectual disabilities, including those with chronic conditions such as Alzheimer’s disease and related forms of dementia. Referrals are accepted from Home Health Case Managers, and while our center is located in Powell County, we serve clients from neighboring counties such as Clark, Estill, and Montgomery.

What We Offer

• Social Activities: Our center offers a variety of daily activities including supervised exercise programs, crafts, outings, gardening, and relaxation techniques.
• Health Monitoring: Skilled nurses and LPNs are available on-site to provide assistance with meals, medication, toileting, and health monitoring.
• Independence Preservation: We empower individuals to maintain control over their activities, boosting self-esteem and positive relationships.
• Convenience for Caregivers: Families benefit from respite care, reduced stress, and increased socialization opportunities for their loved ones.

We are licensed through the Office of the Inspector General and Medicaid, offering Medicaid coverage for qualified patients.

Active Day Services offer a multitude of advantages for adults, aimed at prolonging their ability to reside in their own homes or within the community for as long as possible. These benefits include:

1. Safe Environment: Active Day Centers provide a secure setting for adults dealing with Alzheimer’s disease, dementia, or mobility issues, reducing the risk of wandering, falls, or self-injury that may occur when left unsupervised.
2. Social Interaction: Participants engage in social activities and enjoy companionship, which has been shown to enhance both mental and physical well-being, promoting a sense of belonging and reducing feelings of loneliness.
3. Fall Prevention: Through tailored exercises designed to strengthen muscles and improve balance, Active Day Services help reduce the risk of falls among participants, contributing to their overall safety and mobility.
4. Improved Sleep Quality: Consistent engagement in physical exercise and social interactions during the day can lead to better sleep quality, providing participants with restorative rest and improving their overall health.
5. Caregiver Support: Active Day Services alleviate caregiver stress, burnout, anxiety, and depression by offering respite care and support for family members and caregivers. This allows caregivers to attend to their own needs and responsibilities while ensuring their loved ones receive quality care and stimulation during the day.

Clients can choose private pay or opt for Medicaid coverage if eligible. We offer flexible scheduling options to accommodate individual needs.

Transportation to our center is available through Foothills Express, providing non-emergency medical transportation to Medicaid clients in select counties.
