Foothills Active Day Center

Participants spend their days connecting with others, taking part in meaningful activities, building and maintaining skills, and enjoying opportunities to be involved in their community. Active Day is about more than care. It is about friendship, connection, confidence and belonging.

Participants have opportunities to build relationships, try new things, enjoy familiar activities and be part of a group where they are known and valued. For some, Active Day provides socialization and companionship that may be difficult to find elsewhere. For others, it provides structure, routine and continued opportunities to practice skills and maintain independence.

At the same time, families and caregivers have the peace of mind that comes from knowing their loved one is supported in a caring, dependable environment.

Located in Clay City, Kentucky, Foothills Active Day Center serves Powell County and surrounding communities, including Estill, Clark and Montgomery counties.

Foothills Active Day Center serves adults age 21 and older who may benefit from daytime support, supervision, personal assistance, health monitoring, socialization and meaningful activities.

Participants may include adults with a wide range of disabilities, health conditions and support needs, including but not limited to:

• Intellectual and developmental disabilities
• Autism spectrum disorder
• Down syndrome
• Cerebral palsy
• Physical disabilities
• Mobility limitations
• Neurological conditions
• Traumatic brain injuries
• Alzheimer’s disease and other forms of dementia
• Memory loss or cognitive impairment
• Chronic health conditions
• Conditions that require regular health monitoring
• Difficulty with activities of daily living
• A need for assistance with meals, medication, toileting or personal care
• A need for supervision or a safe, structured environment
• Limited opportunities for socialization or community involvement
• A need for continued support with communication, social, daily living or independent living skills

Participants do not have to fit into one specific diagnosis or category. Every person is different, and our staff works with individuals, families and case managers to understand each participant’s needs, abilities, interests and goals.

There is always something happening at Foothills Active Day Center.

Each day is designed to keep participants active, engaged and connected while providing the support they need. Activities vary throughout the week and are adapted to individual abilities, interests and choices.

Participants may begin the day visiting with friends and getting settled in before taking part in morning activities. Throughout the day, there are opportunities for movement, creativity, learning, socialization and relaxation. Meals, personal care and health needs are supported as needed, and some days include community outings, special events or seasonal activities.

A typical day may include:

• Games and group activities

• Supervised exercise and movement
• Gardening
• Life skills activities

• Seasonal activities and celebrations
• Opportunities to practice and maintain independence
• Social time with friends

• Quiet time and relaxation

No two days are exactly the same. Our goal is to make each day meaningful, enjoyable and engaging while helping participants build relationships, maintain skills and be part of their community.

Foothills Active Day Center combines meaningful activities and social opportunities with the health and personal support some participants need during the day.

Our team includes nursing staff who can provide health monitoring and assistance based on each participant’s individual needs and plan of care.

Support may include:

• Health monitoring
• Medication assistance and monitoring
• Assistance with meals
• Assistance with toileting and personal needs
• Support with activities of daily living
• Mobility assistance
• Nursing services and supervision
• Coordination with families, case managers and other members of the participant’s support team

Our goal is to help each participant remain as independent and involved as possible while providing the support needed for a safe and successful day.

Caring for an adult family member can be rewarding, but it can also make it difficult to manage work, appointments, errands and other responsibilities.

Foothills Active Day Center gives families and caregivers dependable daytime support while their loved one enjoys a safe, active and social environment.

Active Day can give caregivers time to:

Families remain an important part of each participant’s support team, and we value ongoing communication with caregivers.

We work with families as well as professionals and organizations throughout the community.

Transportation options may be available for eligible participants through Foothills Express and other transportation resources, depending on the individual’s location, eligibility and transportation needs.

Foothills Active Day Center accepts private pay and participates in Kentucky Medicaid. Payment and coverage depend on the participant’s eligibility, authorized services and individual plan.

Private-pay services are available for individuals who do not use Medicaid or who choose to pay privately. Contact the center for current rates and scheduling information.

Services may be available through Kentucky Medicaid Home- and Community-Based Services (HCBS) waiver programs. These programs help eligible individuals receive services and supports in their homes and communities rather than in institutional settings.

Foothills Active Day Center may serve participants through the Home and Community Based (HCB) Waiver, Michelle P. Waiver (MPW), and Supports for Community Living (SCL) Waiver, depending on the participant’s age, eligibility, authorized services and person-centered plan.

Because each waiver has different requirements and covered services, eligibility is determined individually. The Home and Community Based (HCB) Waiver and Michelle P. Waiver (MPW) serve eligible participants age 21 and older, while the Supports for Community Living (SCL) Waiver may serve eligible participants beginning at age 18. Families and case managers are encouraged to contact us so we can review the individual’s waiver and authorized services.

What is Adult Day Health Care?

Adult Day Health Care provides daytime services in a community setting for adults who may need nursing support, personal assistance, supervision, structured activities or opportunities for socialization.

Foothills Active Day Center combines health and personal support with meaningful activities, friendships and opportunities to stay active and involved.

Who can attend Foothills Active Day Center?

We serve adults age 21 and older with a variety of abilities and support needs. This may include individuals with physical disabilities, intellectual or developmental disabilities, Alzheimer’s disease or dementia, chronic health conditions, mobility challenges or other needs that make daytime assistance beneficial.

Does Medicaid pay for Adult Day Health Care?

Kentucky Medicaid may cover Adult Day Health Care for eligible individuals, including through certain Medicaid waiver programs. Coverage and authorization depend on the individual’s eligibility and plan of care.

Can I pay privately?

Do participants have to attend every day?

Scheduling is based on the needs of the participant, available services, authorization and program availability. Contact us to discuss scheduling options.

How do I make a referral?

Start by calling Foothills Active Day Center at (606) 663-0794. We will talk with you about the participant, their needs, payment or Medicaid coverage and the next steps in the referral process.

Families may call directly. Referrals are also welcomed from case managers, healthcare professionals and community organizations.
