We welcome patients with Medicaid, Medicare, and most private insurance plans, and we also offer a sliding fee program based on income, providing discounts.
Kentucky River Foothills Development Council, Inc. is committed to providing patients in Powell County and the surrounding areas, access to affordable health care.
Since 2005, KRFDC has been providing a diverse array of healthcare services. In 2011, we introduced the Foothills Health & Wellness Center in Clay City, boasting a sizable 3,000 square feet area and four fully equipped exam rooms.
Foothills Health and Wellness Center extends its services to individuals experiencing homelessness, those at risk of homelessness, and individuals who are underserved. Homelessness is defined as lacking permanent housing, including residing on the streets, in vehicles, staying with friends or relatives, or living in inadequate housing conditions. Additionally, individuals and households spending more than half of their income on rent and utilities may also qualify for our services.

Payment Options
Our program operates on a sliding fee scale and offers assistance with payment plans to accommodate our patients’ financial situations. We accept Medicare, Medicaid, and most private insurances. Rest assured; our program provides care to patients regardless of their ability to pay.

We strive to accommodate your schedule when booking appointments. When calling to schedule, please inform the receptionist of all your symptoms and concerns. If you need to cancel your appointment, kindly notify us at least 24 hours in advance. We recommend arriving 30 minutes prior to your appointment time.
What to Bring to Your First Appointment
For your initial visit, please bring personal identification (such as a driver’s license or picture ID), insurance cards, and the name and address of your previous primary care provider, if applicable. Additionally, remember to bring all currently prescribed medications. If you cannot bring the medications, please provide a list including over-the-counter medications, herbal remedies, and vitamins.

Upon diagnosis, if specialized care is deemed necessary, your provider will facilitate a referral to a specialist. Our dedicated referral coordinator will manage the referral process and arrange appointments on your behalf. Should you need to reschedule, kindly contact the specialist’s office directly. Transportation services are available for your convenience to ensure you reach your appointments seamlessly.
