Community Health Worker
Location: Health & Wellness Center | Powell County
Job Description:
The Community Health Worker (CHW) provides outreach, education, and care coordination, linking clients to vital community resources and supporting front office functions that keep the clinic running smoothly.
The CHW serves as the first point of contact for patients, assisting with check-in and check-out, connecting patients to healthcare and social services, and helping reduce barriers to care. This position blends clerical support with patient-centered engagement to improve health outcomes for vulnerable populations. Agency policy regarding drug and alcohol testing and criminal background check apply.
Education, Experience & Training: High School Diploma and/or GED required. At least 2-3 years of experience in health care setting required. CHW certification (or willingness to obtain certification through the Kentucky Office of Community Health Workers within 12 months of hire) preferred. Certification in CPR is preferred.
