Community Health Worker
Location: Health & Wellness Center | Clay City
Job Description:
Compensation: Full-time (37.5 Hours Per Week)
Non-Exempt Position
$15 – $17 per hour
(Contingent on experience)
Position Summary: The Community Health Worker (CHW) provides outreach, education, care coordination, and linkage to community resources while also supporting front office functions that ensure smooth clinic operations. The CHW serves as the first point of contact for patients, assisting with check-in and check-out, connecting patients to healthcare and social services, and helping reduce barriers to care. This position blends clerical support with patient-centered engagement to improve health outcomes for vulnerable populations.
Education, Experience & Training: High School Diploma and/or GED required. At least 2-3 years of experience in health care setting required. CHW certification (or willingness to obtain certification through the Kentucky Office of Community Health Workers within 12 months of hire) preferred. Certification in CPR is preferred.
