Non-Exempt Position/37.5/40 Hours Per Week
$22.77 per hour

Position Summary:  The SSRE Recovery Navigator provides targeted case management to clients and connects them with community resources. This role involves care coordination, health education, interdisciplinary collaboration, consultation, and administrative duties. The Recovery Navigator works closely with the client’s primary care provider and interdisciplinary treatment team members, including medical, nursing, administrative specialists, and targeted case management personnel, to ensure comprehensive and
client-centered care.

Educational Requirements: Bachelor’s degree in a behavioral science field (e.g., psychology, sociology, social work, family studies, human services, counseling, nursing, behavioral analysis, public health, special education, gerontology, recreational therapy,
education, occupational therapy, physical therapy, speech-language pathology, rehabilitation counseling, or faith-based education) as specified in 908 KAR 2:260. Experience: Minimum of one (1) year of full-time professional experience serving the targeted population in a human services setting. A master’s degree in a behavioral science field may substitute for the required experience.
Training: Successful completion of a minimum of twelve (12) hours of core targeted case management training covering essential functions and guiding principles, as mandated by 908 KAR 2:260. Ongoing continuing education to maintain certification, as
required by state regulations.

Other Requirements: Must have a valid driver’s license and the flexibility and willingness to changes in schedules and duties as needed according to staff availability. Must be willing to travel the service area as needed.  Agency policy regarding drug screens and criminal background checks applies to this position.
